did not receive Pan card as i had applied for change name

Dear Sir,

My acknowledgemnt no. for change of name in pan card application is 062179700018472 and name is Amruta H.Mehta..filed on 13 septemebr 2014. i had inquired to the pan card office and they said that application is not uploaded yet from where you had applied .i have inquired to Karvi ahmedabad office as well as hyderabad office however they all are hang around me from ahmedabad office is saying that i had sent it and hyderabad office is saying that i did not receive your application though ahmedabad office provided the blue dart courier no. which is 14320000122 and received by mr. karthik at hyderabad office.Zara,Jyothi,Phany ,shrey from hyderabad karvy offices are hangaround me as i have made 1`5 calls in 2 days.

Due to not getting the change pan card i am not able to open any account as it is mandatory.kindly do needful and take immediate action.
