Not providing satisfactory services

I have taken Broadband connection from M/s. Tikona Digital Network Pvt. Ltd. on 28/03/2015. My used id is 1112845530. This plan is taken for 12 months at amount of Rs. 4500/-.
I have registered first complaint id no. 1-3439034728 dated 05/4/2015 for not working of internet connection. on 9th, 11, 28 April was follow up for the same and services was activated with resolved of technical issues. But again in short span the technical issues was created and till date the issue is not fully resolved by the above agency. I am many times (from last 3 months) given in writing through e-mail (approx. 15 nos.) and also on their customer care no. verbally (approx. 25 calls) to Tikona for disconnect my connection but they are not neither disconnect the services nor resolved the technical issues (not providing satisfactory services). during last 5 days i am continuous calling to attend the technical issues or disconnect the connection but they have not attended the issues on permanent basis. You are kindly requested to help me for refund of Rs.4000/-.
