ICICI Prudential Life Insurance Policy is not issued even after then encashed the amount

Name of company/service: ICICI Prudential Life Insurance

I am manjunath, subscribed for ICICI Prudential life insurance through offline, One of the executive has collected the cheque of 25,0000 bearing the no 559159 in name of ICICI Predential Life Insurance Pvt Ltd(crossed and it is Axis bank)and they have taken sign in the application and they have given the application no as 4101763. Unfortunately i have not taken the acknowledgement and contact details of the executive of ICICI insurance and i am not able to contact them.

After 10-15 days the cheque is en-cashed. I tried numerous times contact the customer care and they are not able to trace policy by the application no or by the contact details of mine.

The telephone no of the branches of ICICI are wrongly published online ane even the customer care are not have any idea of that.

Please guide me to resolve it. i am planning to complain with police.
