ICICI Bank charges 20,000 for credit card I have never applied for

I have zero balance savings account with ICICI bank. Last month, my account was debited 20,000. When enquired at the branch, I was told that this debit was for pending charges I had accumulated from 2007 to 2014 on a credit card.

I have asked for complete transaction record. They sent me the transaction record for the entire period (almost 7 years) and there is not even one transaction in my name. All the transactions are their annual credit card charges and late fees.

I have never applied for this credit card. When requested for copy of my application form, they promised to deliver it by 27 april 2014. they were unable to deliver it and after repeated mails, they have not sent me the application forms.

At the same time, the credit card department do not specifically state that they do not have my application forms with them.
