Dear Sir,
With due regards, I intend to convey that i am working with Religare Aviation Engineering Ltd, New Delhi and a health card with policy no.-111700/48/2012/534 of DHS TPA (India) Pvt LTd. has been provided by the company and sum insured for entire family is Rs 200000 as mentioned in my account on DHS Website.
My mother had got operated for Stone in Gall Bladder on May 7, 2012 in Dr. Ruban Memorial Hospital, Patna which is included in DHS hospitals list.Amount charged by the hospital was Rs 26018 for 2 days and i have already submitted the Bill and Discharge Card of hospital through my Company to DHS to claim the amount.But when i talked to DHS for reimbursement, the company declined by saying it is not in your policy.
Sir may i request you to take quick action on this issue!
