service barred because company had entered my name wrongly

Location/place: Vijayawada.

Name of company/service: tata docomo

Hello,

i have taken a tata docomo prepaid sim card from a retailer and had submitted relevant documents .
My mobile number is 08712251897

The company people had entered my name wrongly in their records.

but they had entered my address correctly.

But they disconnected my service .It is totally unfair from the company as it is their internal problem.pl restore my service as there is no fault from my side.
