Sandisk 16GB memory card formating problem

Sir, I am H.R.Deb from Raipur, I have purchased one sandisk 16GB memory card in the month of September 2013. Date of manufacturing was August 2013. I got problem in the month of January 2014. I have complaint one week later at sandisk service center at Raipur.(C.G).
They advised to contact online support, Till date i am trying to solved my problem thru online and mailing after mailing but they have not solved the problem but diverted thru questioning which is not related to the problem.It means nither they wanted to solved the problem or replacing my memory card which is in the warranty period.

Dear Sir,

This is in reply your mail i wish to inform you that since bill has been misplaced, i don’t know the exact date of purchase. I have only warranty card (cover) of memory card. And date of manufacturing of this card is Aug-2013 and month of Import is Aug-2013. which is printed on the card. Hence it is my request to leave the sales and purchase procedure and come to the exact technical fault.
And once again requesting you please solve the problem of my memory card at the earliest.

-What is the properties of card showing in computer ? Free space used space and total capacity

-Have you checked the card on any other device or a computer?
-How do you connect the memory card to the computer?
-What is written on the slot where you insert the memory card?
-Do you have any important data saved on the memory card?

Dear Sir, I purchased one 16GB memory card 6month before, but now suddenly it is asking for formating, when i going to format it is showing formating did not completed. PRODUCT CODE-SDSDQM-016G-B35
