As a customer of cigna ttk since last four years I have two policy as proposer named Usha Umesh pawar.which is after marriage name. Which is PROHLT158500255 and PROHLR-150008706 respectively..
Recently some genuine reason which I would not want to disclose, I hv changed my name as Usha Kantilal Modi which is my maiden name indicates on all my School certificates. For futer purpose I requested to cigna to make correction in my name in the policy. For that I hv provided following document as correction proof are as, Gazette issued by Maharashtra govt., Aadhaar card, pan card and School certificate. Besides this they are not making the correction and insist me to provide divorce certificate which I cannot provide. So please look after the matter and support me to make correction to b continue with my policies which will be due in very short period of time in next month.
