no revised pension in a/c

Name of company/service: Punjab National Bank,Jind

After the death of my husband in Nov.2009, I am recieving the family pension. Till Sept 2011, I was receiving pension with revised pension. But after that it is coming at earlier rules. My all efforts are in vain.
