My Mobile Service Are Deactiveted

Name of company/service: reliance cdma

Dear sir or Madam,
My name is a chandrakant thakur and my mobile no is-9323858950.I have started reliance CDMA postpaid service from the day 11/04/2013.But they have deactivated my service from 19/4/2013.Since 19th april i am visiting reliance gallery they told me the verification of my address is not done please re sumited the form and i have again sumited a CAF form. After 7 days are over but still my service is not activated.I have again and Again going reliance gallery,i have call customer care many time,I have send many emails to customer care but they cant help me.
Please help me .
