money not received from atm still my account debited

Name of company/service: ICICI Bank

Hello,
I am Pooja Joshi from Mhow Indore using ICICI bank for my salary account and my account no is 144101501997 and my branch IFSC code is ICIC0001441 Palasia Indore.
I made a transaction from SBI ATM MCTE MHOW Branch (ATM_ID – S100030023014) on 5-July-13 for 4000 Rs. But after entering amount it directly came at main screen but amount is debited from my account and the transaction no is 3934 . I Submit an appilication form on 6-July-13 at ICICI Bank MHOW Indore branch but no response received yet. Please its a humble request, do some needfull.
