Fake cancellation ID of credit card enhancement pack

I got my savings account in Standard Chartered Bank from the month of February 2013. I kept getting calls in the initial days regarding address confirmation,phone number verification etc. In march I got a call for credit card enhancement pack which had monthly payments of 840 rupees. I refused it several times and still some person on behalf of SC bank kept calling me and said you have to take this offer there is no other go and said’ I will cancel it immediately once they activate it’. So I accepted it and that representative gave me a cancellation ID as well. Till the month of July I was under the assumption that the card enhancement pack is cancelled.
On July 2nd when I accidentally happened to check my cards section online I discovered that I had a due of 4000. When I went to the nearest branch they said ‘ you have to solve the credit card issues only through phone banking ‘ . Then I called the phone banking I got a shock , the cancellation ID I had was actually the service id and my pack was still active with interest building on and on. I realised then that I was fooled.
The credit card executive says me that ‘you never called the bank to cancel your pack ‘

COMPLAINT 1: A person on behalf of SC bank calls me and says your pack is cancelled and gives me a ID. Why should I call the Bank? I was fooled.

After placing the cancellation request for the pack and reminding them thousand times by calling helpline they cancel it after a months time. And they say they are not ready to refund the amount.

I placed a complaint with the bank and still they haven’t solved the issue and I forced to pay 5500 rupees.

Complaint 2: I want you all to find out that executive who cheated me and take necessary actions and the Bank refunds my money .
