Dear Sir,
Max New york life had wrongly presented twice the ECS towards the premium for which the payment had been made by the undersigned through cheque . The above transactions resulted in a penalty being imposed by my Bank for Rs 1235/- .
I am seeking reimbursement of the above said amount .
