disconnection request for broadband not filed and charges are leavied on next month bill.

I have submitted a disconnection request for broadband services to Bsnl telecom Gwalior as on 23rd june. however, for the submission they have not given me any reciept for the same, and not even made me aware for the same, Now on 13th of july when I got to them, they say there is no such kind of request filed for your connection, moreover when I tried to inquire about the written application form which I submitted, they say we are not able to find any such application , I do have the scanned copy of the application, and I am fed up roaming between there departments…
now they say we would not be able to rebate the charges for the month of july since there is no such request filed under your connection.
what should be done for the same, please help.
