Credit card issue with wrong name by SBI

Location/place: Gurgaon

Name of company/service: State Bank of India

I apply for SBI credit card in last month (application No.1516860001286) and submit the all required document to concern person at my office. My name is mention on hard document is Pawan Mishra (PAN card and DL), but SBI is issued credit card with name of Rawan Mishra. This reason courier company is not given card to me. Yesterday I was got PIN no. I am fallow up from last weeks and raised complaint at customer care but till now we have not get any solution for this issue. I am really frosted. Please help us to resolve this issue.
