Money getting deducted from my account

Every month i am facing the same problem.
i will recharge for 2g for 98 rupees and the amount will get deducted from my account when we call up customer care they say that i didnot do the recharge and will deduct the amount from my main balance.
till now i lost around 300.to 400 rupees. Customer care will tell that they will resolve the issue but never refund the amount 7353801213.
