Sub: CLAIM OF my daughter UNDER COMPLIMENTARY PERSONAL ACCIDENT INSURANCE FROM TATA AIG CREDIT FOR INR 2 LACS
Dear Sir / Madam,
My daughter met with an fatal accident on 6-10-2011 while going to office at airport circle, Ahmedabad. She was working as an assistant manager for Indigo Airlines. After 5 days in coma at sterling hospital she expired on 10-10-2011…
She was using your TATA credit card no. 5126 **** **** **** with credit limit INR 50,000/- issued on 08-2011 valid up to 08-2013.
According to the Brochure enclosed with the TATA credit card “The power of one is in your hands“ entitles complimentary Personal accident Insurance covers upto INR 2 Lacs from TATA AIG in case of loss of life resulting from rail, road, or other accidents.
To avail insurance benefits ensure that the card has been used at least once for purchase / cash made within 90 days preceding the events.
Claim Procedure:
1. A preliminary notice of claim must be sent to TATA AIG General Insurance Co. Ltd. At the given address with 30 days of death to record the claim.
2. A claim form provided by TATA AIG general Insurance will have to be completed and returned along with additional documents as mentioned below:
a. Original / attested post mortem / coroner’s report / attested copy of FIR / Police Inquest report where applicable
On 31-10-2011 I personally Ajaykumar P Patel, father of ____________ visited TATA AIG General office at 10th floor, abhijeet III, Opp. Mayors bungalow, law garden, Ahmedabad there I met Branch Claim manager B. Anjaneyulu and gave him the complete bunch of preliminary notice, death certificate, FIR copy, Newspaper cutting of the accident incident, there then he told us that he will sent all the papers to Bombay office.
Again I visited same above said Ahmedabad office on 15-11-11 & submitted other documents as sterling hospital summary report, sterling hospital death report, Death certificate, post mortem report of civil hospital to B. Anjaneyulu.
Mr. Krishna Mallya from Bombay office called me on my cell no. 9327053611 and told me that he is received all the documents I gave to Mr. B. Anjaneyulu and going to sent all the related documents to me. Once I receive the documents I have to submit the document to the above said office of TATA AIG General Insurance Co. Ltd., Ahmedabad.
Received documents from Bombay office and I went back to the above said Ahmedabad office, there I met Mr. Birju Pandya on 25-11-11 as advised by claims manager Sachin Jangra at Ahmedabad office. Here, Mr. Birju Pandya sends back all the documents back to Bombay office.
When I documents where received in Bombay office, I got a call from Savita Archarya and she confirmed that all the documents are received by her and will handle our case. Now, in the conversation she is saying that we are not entitling for the claim as you have not ask for it in the written. So, I explained her entire procedures I have gone thru submitting the documents thru no. of the claim managers. Even I mentioned the in the brochure of the credit card page no 9, there its already mentioned about the complimentary accidental insurance in the case of rail/road/other accident incident amounting INR 2 Lacs. Then she asked me to send her the photo copy of the brochure.
Now, as per the request I took the photo copy of the brochure and credit card and send it to Mr. Birju Pandya, Ahmedabad office (personally hand to hand submitted the photo copy) and another copy to Savita Archarya, Bombay office with the presence and knowledge of Claim Manager Mr. Sachin Jangra.
I tried number of times calling to Savita Archrya, But I am getting no reply from Bombay office and then after contacting Sachin Jangra and regular follow up on 6-3-12 TATA AIG LIFE office at Ahmedabad, as advised by Mr. Birju Pandya again I filed death claim documents of my daughter including the following new documents of Panchnama, Inquest panchnama, Post mortem report, Hospital Statement given to the police, her Passport, Parents passport, and bank statement, duly signed and attested and submitted by me to Mr. Birju Pandya.
Still upto this date I have not received any response / reply / written documents / letters / calls from the Insurance office (neither Ahmedabad nor Bombay)
Guide me what to do, and how can I proceed further on it. I am being obligatory to go to consumer dept. with this case, to get an official response from the insurance company…
I will appreciate a written letter precisely detailing about the update and status of my claim from the insurance office.
