Fraud process mobile insurance without my signature and inform

I , Pinky halder, 9836667222, residng at 23 simla road P.o manicktala,Pin Code-700006,

I had taken a IPHONE 5S 16 GB mobile from Systematix Media ,via Bajaj loan a/c no.-410DPF26180079

But reason is that, After some days I received Insurance KIT at my postal address. It was mobile Insurance(Insurance Loan Account No : 410DFS26185466) . That time I went to Phone Shop where I had taken my phone and said I purchased only phone. I have not purchased any insurance for this mobile.
I didn’t any signature for any type of insurance paper.
so how is possible without my signature & information ?

Thats why I m sending many mail. but want from them proof copy where i had given signature for this Insurance.

but they were unable to provide.

and Insuranse amount showing Rs.2299 and right now with penalty show Rs.20785 this is totally fraud.

sir please look this matter asap.
Long time this purpose has been mentally harassed and tortured for me.

AND I had sent this mail for Two Times in 3 months .

My phone (Loan account number 410DPF26180079 ) was not Insurance Cover .

I didn’t any signature that type of insurance.

Which time I had purchased my phone that time No body said to me that was Insurance Covered.
