Sub: Non-Receipt of Policy.
I would like to inform you that I have applied for LIC Amulya Jeevan Policy on 14.02.14 of Rs.50 lacs and after submitting the financial papers & medical examination reports, your branch had issued me policy bearing no. 895331506 date of commencement on 29.03.14. Enclosed herewith copy of same for your reference.
After getting approval of the same policy, I had made additional application for the same amount & same terms & conditions & same had been acknowledged vide proposal no. 336 & premium receipt no. 343 dt: 12.05.14 & also cheque for same had been cleared in my account on 16.05.14.
In this matter, I had already written Reg. AD letter dt: 14.07.14 received by your office on 16.07.14. It is really surprising & shocking that inspite of making request letter to clarify the status of policy, there is no response from your side. Enclosed herewith copy of same for your reference.
In the given circumstances, if this matter is not resolved within 2days tiome from receipt of this letter then , I am left with no other alternative but to inform IRDA & Consumer Court/Forum at your cost & risk.
Sir inspite of our personal contact with D.O. & branch, there is no response from their side from 12.05.14 till date either orally or in writing.
