Bill generation before card recieval

Location/place: Pune

Name of company/service: ICICI Bank

Hi,
Some times back I had applied for ICICI credit card, but after submitting all the documents & gone through all the verifications after 1 month, I have not recieved my credit card. And the worst thing is ICICI had generated monthly statement for the card with joining fees as the due amount and mailing me again & again. They are not even telling how to pay the amount without knowing credit card no.
